Title:  Social Worker                                            Report to: Executive Director           Location: Port Clinton OH

 

 Opportunity

 

The Social Worker is responsible for providing professional social services to individuals, families, and groups to enhance well-being, address social and emotional challenges, and connect clients to appropriate services.  This role will also support staff well-being and advise appropriate methods to advocate for clients while being sensitive to the client’s mental health needs. This role involves ensuring that clients’ voices are heard, their rights are protected, and they have access to appropriate resources and services. The Social Worker works collaboratively with clients, community partners, and service providers to address challenges, remove barriers, and promote positive outcomes.

 

      Responsibilities

 

  • Complete comprehensive psychosocial assessments to identify client needs, strengths and risks.
  • Collaborating with agency Advocates, develop appropriate service plans for clients.
  • Provide crisis intervention as needed.
  • Monitor client progress and adjust services as needed.
  • Maintain accurate and confidential case records, documentation, and reports.
  • Conduct outreach and education to increase awareness of client needs and available services.
  • Provide education and support to clients and families regarding coping strategies, resources and services.
  • In conjunction with Grandlove staff, lead all the current support groups and assess the effectiveness of these groups.
  • Participate in training, supervision, and professional development activities.
  • Other job responsibilities may be added from time to time in support of the evolution of the FAC.

 

      Skills and Experiences for Success

 

•       Bachelor’s or Master’s degree in social work from an accredited program (MSW preferred). Current state licensure required.

•       One to three years’ experience working with vulnerable populations or in advocacy, counseling, or case management roles.

•       Strong communication, negotiation, and interpersonal skills.

•       Knowledge of social work principles, ethics and relevant laws and regulations.

•       Knowledge of community resources and support services.

•       Commitment to client-centered care.

•       Ability to work independently and as a part of a multidisciplinary team.

•       Strong organizational, time management, and multitasking skills.

•       Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and database management.

•       Flexibility to work evenings or weekends, as required by program needs.

•       Ability to travel as needed.

•       Highly organized: able to effectively manage many tasks simultaneously.

•       Reliable transportation for community-based work.

•       Other duties as assigned.
