Essential Functions:
The primary contact for participants in the Brother’s United Program, the Case Manager/Coach will guide the participant through the program and provide continued support in the initial stage of the program and throughout the program. The Case Manager/Coach will manage an active caseload of participants. Responsibilities include: recruitment of participants, entering them into the ETO system, participating in weekly meetings, completion of all required contacts and assessments in ETO, weekly case management meetings with participants, linkage and referrals to community resources, completion of all case notes and follow up to ensure participant completion of the program.
Qualifications:
- Must be self–motivated.
- Must be able to work independently without daily supervision.
- Must possess strong organizational skills.
- Must be able to provide outreach to the community.
- Must have excellent written and oral communication skills.
- Must have the ability to coordinate services based on the participant’s needs.
- Must be able to perform all tasks necessary to achieve the organization’s mission.
- Must provide timely intervention for program participants when needed.
- Ability to facilitate evidenced based curriculum with the exception of MRE.
- Ability to motivate others toward achieving goals.
- Ability to maintain participant’s files including case and progress notes.
- Ability to maintain data system updates
- Ability to assist with collection, preparation, and submission of weekly and monthly enrollment reports.
- Able to encourage teamwork and provide leadership to program participants.
Education: A Bachelor’s Degree or any combination of training and experience equivalent to 2 years of college.
Preferred Qualifications:
Previous experience working in a Fatherhood Program.
