Mobile Meals of Toledo (MMoT) is a nonprofit established in 1967 that was founded to support the greater Toledo area seniors and the medically fragile with access to nutritious meals, social connection to enable our neighbors to age in place and enjoy a healthy, safe and independent life.
The Client Services Care Coordinator: Assists with the agency's operations, starting when a potential client, caregiver, or Case Manager contacts the agency for food service and meal sign-up. This position is responsible for interviewing new home-delivered meal clients, assessing current clients, and determining client eligibility to receive services. This position will also be the lead intake specialist and care coordinator for active clients enrolled in home-delivered meals.
Once a client starts services, the CSCC should follow up with clients regularly to ensure they have access to the appropriate supportive services to enable them to stay independent and remain in their homes for as long as possible. This position will have frequent contact with clients, agency personnel, case managers, discharge planners, and the public. A positive, forward thinking and cooperative image and attitude are crucial for this position.
Work performed in this position will be of a complex, professional, and confidential nature. Work requires considerable understanding of seniors and handicapped individuals, and an aging network with attention to detailed written documentation, customer service, interviewing, and communication skills. Work performed is under the general supervision of the Operations Manager and in cooperation with other Mobile Meals staff. but it will require a significant amount of independence and self-direction.
Primary Overview of responsibilities for home-delivered meal program:
· Responsible for interviewing new home-delivered meal clients, assessing current clients for additional support needs, and determining client eligibility to receive services.
· Responsible for producing accurate detailed client information for client databases, including income verification. This includes reviewing and revising interview paperwork, referral applications, and reassessment forms; providing required funder paperwork; and relaying appropriate paperwork and information to the department across agency.
· Performs other departmental and agency-related functions as necessary. This includes attending departmental and agency meetings as necessary, serving on departmental and agency committees as necessary, attending and assisting with departmental and agency special events as necessary, and performing other assignments as necessary.
• Perform additional duties as needed to maintain the efficient operation of the office.
Special Projects for supportive services:
· Coordinates the Pets Assisting the Lives of Seniors (PALS), monthly pet food assistance program (intake, routing, pet food ordering, satisfaction surveying, and documenting best practices with other Meals on Wheels).
· Coordinates the Durable Medical Equipment HUB in partnership with the Ability Center (intake, calls for requests, re-ordering DME and hygiene products, managing inventory, documenting procedures, and reporting monthly to the partner).
· Coordinate work and schedules for social service interns with local Universities.
· Other client-centered projects as needed.
Qualifications:
• Bachelor's degree and/or three years’ experience in a nonprofit, social service setting
• Excellent telephone and interpersonal skills
• Demonstrated customer service skills while dealing with a diverse population
• Excellent computer skills for record-keeping, reporting, and data entry purposes
• Computer proficiency; knowledge of electronic health record systems or other Customer Experience systems (CXM/CRM) and Microsoft Office Suite
• Experience creating reporting statistics for the program and meeting deadlines
• Able to remain calm and professional in a stressful situation
Physical & Cognitive Demands: This position is generally set in a comfortable office environment. There are times on a monthly basis this role requires work outside the office. The volunteer field work may occur during or after business hours out in the community. The role frequently interacts with others. Community events range from a few hours to an entire day in indoor and outdoor environments. This role requires seeing, hearing, sitting, walking, driving, talking, writing, walking on uneven surfaces, standing for extended periods of time, depending on the situation.
Job Type: Full-time, hourly
The regular work schedule may include very limited evenings, weekends, and early mornings (the weekday schedule is adjusted accordingly) from time to time. The work environment is generally comfortable, and most work is indoors, although the position may require work outdoors with exposure to seasonal weather conditions for special events, meal delivery, etc.
Pay range: $18.50 - $22.50
Benefits: Medical, Dental and Vision coverage. Short-term and Long-term disability. 401k with 3% match after 6 months. Two weeks paid vacation, two weeks personal time off, and 10 paid Holidays.
