The  East Toledo Family Center (ETFC) is  the  anchor  organization  in  the  East  Toledo  community. We  provide   programs  and  services  to  support  individuals  and  families  from  birth to  seniors. We  educate, engage  and empower  our  families  and  community  to  make  their  own  change. The  Pathways  Community  Health  Workers (CHW) are front line public health workers who have  a  close understanding  of  the community they serve. This trusting  relationship  enables  them  to  serve  as  a  liaison  between  health/social services  and  the community to facilitate  access  to  services  and  improve  the  quality  and cultural  competence  of  service  delivery. PRINCIPLE DUTIES & RESPONSIBILITIES:

  • Conducts  home  visits  with  clients
  • Develop  strategies  to  positively  engage  pregnant and  adult clients  in  voluntary  services  through  a  trusting relationship
  • Schedule and  provide  regular home visits with families  and  maintain  an  average  caseload  of  30+ clients
  • Conduct  ongoing  assessments  such  as  Visit  form  and  other  tools  required  by  the  program
  • Implement  research-based  curriculum,  Partners for Healthy Babies & Families,  during home visits
  • Support  and  guide families  in  the  development  of  ongoing  health  management
  • Refers  and  connects  families  to  additional  services  and  other  community  resources  that  could  help  strengthen  the  family
  • Completes  required  documentation  in  accordance  with  program  expectations
  • Meets  bi-weekly  with  team  and  1:1  for  supervision  to  reflect  on  work  with  families  and  review  interventions  and  documentation
  • Completes  all  training  required
  • Assists  with  outreach  through  community  fairs  and  other  events  that  help  build  awareness  of  program
  • Completes  documentation  through  on-line  database  within  48  hours  of  communication/visit  completed
  • Must maintain 40-hour  work-weeks
  • Must  be  willing  to  travel  across  Lucas  County  (mileage reimbursement at state rate provided)

PHYSICAL REQUIREMENTS

  • Must  be  vaccinated  for  COVID-19  due  to  being  an  affiliate  for  Medicaid
  • Locations -- office  environment  and  diverse  community  client  homes
  • Must  be  able  to  provide in-home visits
  • Valid  driver’s  license
  • Travel Requirements-- Must  have  reliable  vehicle, ability  to  travel  to  and  from  family  homes  with   i nsurance   requirements  listed  below (mileage is reimbursed at the state rate)
  • Pass  all  required  background  and  drug  test
  • Will  be  required  to  maintain  car  insurance  fo r  bodily  injury  &  property  damage  Liability  coverage  of  $100,000  each  person/$100,000  each  accident/$50,000  each  accident

EDUCATION:

  • High  School  Diploma  or equivalent
  • Community  Health  Worker  Certification  preferred  but  not   required
  • Individuals  who  are  not  certified  community  health workers  will  have  the  opportunity   to  obtain  their  certification  through  ETFC

QUALITIES:

  • An  ability  to  establish  trusting  relationships
  • Acceptance  of  individual  differences
  • Preferred  experience  and  willingness  to  work  with  the  culturally  diverse  populations  in  the  community
  • Preferred  knowledge  of  prenatal, infant  and  child  development
  • Strong  verbal  and  written  communication  skills
  • Ability  to  maintain  professional   boundaries  when  providing  supportive  services
  • Ability  to  work  collaboratively  and  independent  with  work  schedule
  • Bilingual (Spanish)  ability  preferred
  • Case  management  experience  preferred
  • Due  to  Medicaid  partnership, all  candidates  must  be  vaccinated for COVID-19

PAY RANGE:  $16.00/hour

SCHEDULE: Full-Time,  8-hour shifts,  start time  ranging  from  8am-9:30am,  Monday  through  Friday.

TRAINING:  Orientation  to  the  Family  Center  Employee  Handbook. All  job  related  trainings  as  required.

UNIFORM:    Staff  badge  and  casual  attire  required.
